/ Athlete Registration
All Holland Christian Middle School and High School Athletes are required to register online through a program we are partnered with called FinalForms. This online registration is how all students sign up for school sports teams. For co-op teams and a few non-MHSAA teams, athletes may need to be registered on Holland Christian’s FinalForms registration and another school’s registration separately (meaning 2 registrations). Our community benefits from this registration by streamlining our athletic physical process, managing important data, allowing efficient access for staff, improving communication and collecting pay to participate fees online. FinalForms allows you to complete and sign all athletic participation forms and saves your data from season-to-season and year-to-year… meaning you should never have to enter the same information twice!
You are required to verify your FinalForms information each school year and print the needed annual MHSAA physical form from your account for your MD/DO/PA/NP to perform the required physical. For the 20-21 school year, the MHSAA will accept a 19-20 physical that is on file at school, but a 20-21 health questionnaire (also printed from your FinalForms account) must be completed and also turned in to be stapled and filed together. So, an actual physical exam is not necessary for 20-21 IF a valid MHSAA physical is filed from the 19-20 school year. If you do not have a 19-20 physical on file at school (including all first time athletes), you will need to have the normal MHSAA physical completed by an MD/DO/PA/NP with an individual appointment completed before being allowed to participate on any team. A completed 20-21 MHSAA health questionnaire OR recently completed 20-21 MHSAA physical form needs to be brought to your respective Holland Christian Middle School (6th-8th) OR High School (9th-12th) office. (Zeeland Christian students transitioning from 8th to 9th grade and any transfer students must individually pick up or have a printed copy of their 19-20 completed physical from their prior school to turn in with a 20-21 health questionnaire if this is the chosen option.)
ALL PARENTS of middle school and high school athletes will eventually need to register in order to participate, but are highly encouraged to register in early April of the preceding year (registration opens April 1st). Registering assists athletes to receive communication about summer opportunities for the sports they are registered for. Please follow the prompts to create your account, create your student(s) accounts, sign your forms, and print the required MHSAA physical form or health questionnaire to be completed. Students are required to have a school issued email account. (ZCMS, Unity, and Black River email accounts are accepted along with HC email account.) If the student is a transfer, they will be issued their Holland Christian school email account in early August and will need to wait to complete their portion of the registration until they receive this account. (The parent account fully registers students for specific sports and completes the physical form process apart from the student account.)

/ PHYSICAL NIGHT & IMPACT TESTING
Holland Christian MS & HS annually offers a discounted physical night for the upcoming school year, but due to the COVID-19 virus, the physical night scheduled for April 29, 2020 has been cancelled. Please reference the specific instructions above for 20-21 eligibility. We hope to offer a physical night again for the 21-22 school year and will post it here when available.
Holland Christian High School uses ImPACT as a tool in the athletic training toolbox to assist a safe return for athletes to their respective sport after a sustained concussion. Every athlete who participates in a sport at Holland Christian High School must have an ImPACT baseline concussion test completed prior to the start of the school year, which is then valid for 2 years. Athletes are baseline tested for their 9th grade year and retested for their 11th grade year. New 10th and 12th grade athletes need a “catch-up” baseline test completed to participate. Due to current restrictions with COVID-19, athletes will receive a uniquely assigned link emailed directly from ImPACT “support” to complete their baseline test at home. Athletes will have 90 days to complete the test after it is emailed to them, but need to complete it before participating in any sport.
